The Toolkit
Before we get started:
Download the toolkit flowchart for reference
This toolkit flowchart delineates the step-by-step process, starting from assessing readiness to change, identifying barriers and enablers, and guiding users to the relevant planning documents for developing their tailored program.
Non-physical activity goals
Each step in the toolkit includes downloadable worksheets. While the toolkit is focused on physical activities, there is a set of alternative worksheets for non-physical activity goals available.
Let's get started
The ‘Quick Five’ assessment is a good place to start to get a general understanding of who the survivor of stroke is, and how physical activity and exercise programs may fit into their life and schedule.
- Download the 'Quick Five' Assessment and discuss the questions with the survivor of stroke.
Motivational interviewing has been shown to be more effective than ‘advice-giving’ in terms of promoting change. The aim of using this approach is to enhance your clients’ self-efficacy and intrinsic motivation by exploring and resolving ambivalence and barriers to behaviour change.
- Download the Motivational Interview Guide and use it to guide rather than direct conversations with the survivor of stroke.
After completing the 'Quick Five' assessment, determine if the survivor of stroke is ready to change and start being active.
Have they:
- Said they are ready?
- Identified what they want to focus on?
- Identified personal barriers?
