States of change
For survivors of stroke to adhere to new physical activity/exercise programs, they will often need to change their behaviour. This might be a small change, but for some it will require major change. It’s important to understand if the survivor of stroke is ‘ready’ to change.
- Download the 'Stages of Change' assessment and use the four questions to help you understand the survivor of stroke's 'stage of change'.
After completing the 'Stages of Change Assessment', utilise the scoring algorithm to identify the stage the stroke survivor is at.


If a client is assessed to be Stages 1-3 you will be directed to the COM-B assessment, Motivational Interview Guide and the Behaviour Change Plan. If a client is assessed to be Stages 4-5 you will be directed straight to the Action Plan.
