Module 3: Stroke prevention protocol in your practice

Thank you for attending the live module 2 meeting.

In module 3 you will put what you have learnt into action by proactively identifying patients within your practice who are at increased risk of stroke and systematically implement guideline-based management approaches to reduce stroke risk and provide patient education. Learn about what resources the Stroke Foundation have to help you and your patients’ with the stroke pathway.

Once you have completed module 3, complete the summary sheet with your details and then:

RACGP requirements and getting your CPD points

In order to receive your 40 CPD points, please verify the completion and submission of each of the following program elements.

1. Post-program reflection activity

Please complete the post-program reflection activity of the Acute Stroke Action Program.

2. Formal Assessment

Please complete the learning outcomes formal assessment and then fill in and return the confirmation of program completion form and return everything to the program administrators.

Once you have completed the post-program reflection activity and formal assessment, complete the confirmation of program completion form with your details and then:
