Discharge care planning


Good discharge planning is crucial for successfully reintegrating stroke survivors into the community, and for effectively and efficiently using limited hospital resources. It relies on effective communication between team members, stroke survivors, families or carers, and community service providers, using patient-held discharge care plans and check lists. It also involves the assessment of the carer’s needs and a home visit by an occupational therapist pre-discharge, to ensure safety and the provision of aids and services.
