Learn how to embed the process of Discharge Care Planning in your stroke procedures.
This 1-hour webinar will focus on:
- Compliance under the Living Guidelines for Stroke Management, Acute Stroke Clinical Care Standard, and National Stroke Data Dictionary
- Team engagement in Discharge Care Plan completion and distribution
- Enablers and Barriers, with current site examples of Discharge Care Plans
- Next steps in discharge care planning including working groups and electronic templates.

About the presenters
Lisa Yates is the StrokeLink Coordinator for Queensland. She has a clinical background as a speech pathologist across acute, subacute and community services in both Australia and the UK.
More recently Lisa has been working as an allied health team leader a for rehabilitation-in-the-home service, and in project management, health service performance reporting and health service improvement.
Beth Crane is the StrokeLink Coordinator for Tasmania. She has an allied health and research background, having worked in the community, university, hospital and private sectors, providing care to a variety of patients including post stroke.
Beth is also the Living Well After Stroke Project Officer, delivering the secondary prevention program for stroke survivors in Tasmania.
