My Stroke Journey ensures people have the information they need to take their place at the centre of their care, reduce their risk of stroke and live a good life after stroke.
My Stroke Journey makes it easy for health professionals to deliver high quality information and care planning to patients, family members and carers.
This module will help you:

- get up to date with what’s new in My Stroke Journey
- implement My Stroke Journey delivery at your site.
